This is a FAQ, although I'm not sure if it's answered in the FAQ list...

Presumably you're using mbox folders, as this is very typical behaviour
to them. With mbox folders, the "new" flag as shown in the folder
browser depends on the modified time for a file being newer than the
access time -- ie. the folder has been modified after it has last been
read.
*ANY* read access to such folders will change the last-read time, and
make Mutt no longer mark the folder as new. So, typically this problem
is caused by some other program than Mutt reading the folder file, eg.
a new-mail notified program (eg. biff), a shell's new mail notifier,
backup software being run, etc. etc.
Some new mail notifiers actually take care to reset the last-read time
back to what it was after checking the contents for new mail, but not
all of them do.
