It's a shortcut to your Mail folder, set with the $folder variable
(usually something like ~/Mail, ~/mail, etc.)  Thus +foo would be
the folder in ~/Mail/foo (for example).  '=' is another shortcut, it
behaves identically to +, so it's a matter of taste which you prefer
to use.
