Thanks to all who replied.
The answer is:
setenv COLORTERM rxvt
setenv TERM vt100
turn on ANSI color and vt100 emu in the CRT program
COMPILE mutt with S-lang
Worked first time. Compiling without S-lang, I never got it to work.
