 Even with mono, one can highlight characters with reverse video.

 BTW, ideally parentheses and capitals could also both seen as configurable

 complex for Mutt.

 Without the descriptions, this is even less clear. Just like the Help line

 one could write the key before the description (and here, use several
 lines). So, highlighting wouldn't be necessary (but in addition to that,
 it could also be made optional).
