Jonathan, I'm having the same problems.
I've added the file "android-support-v4.jar" file to a "SupportLib" directory in my Project. I've tried setting the Build Action to both "AndroidJavaLibrary" and AndroidJavaSource" and rebuilding the whole solution and <fragment> is still not available in my axml. For the project properties I have set the "Minimum Android to target" to "Android 4.0". I've downloaded and installed Android 4.0.3, Android 4.0, and several more lower versions. I am using the trial version of Mono for .net.
