Emailing the authors of modules who's last release was 10+ years ago
seems like a sensible first step. Especially if there is a clear list of
next steps (i.e. give it up for adoption). Possibly this should be a
normal part of PAUSE. 

Judging by other responses so far, going further will prove
controversial. At a minimum there would have to be no "fresh" cpan
modules that depend on it. In situations where there is a clear and
superior successor there may be more traction too.
