You didn't load the Xv plugin, I think. Add 'Load "v4l"' to your XF86Config-4 file in the Module section. And yeah, there's better ways to tell a user that he didn't... I guess LVS is "fairly user-friendly, though somewhat picky on how it chooses its friends".
