I am familiar with DJB's writings on queue structure and others. Fundamentally, OpenSMTPD makes use of file system atomic operations similarly to qmail in order to achieve the same goal, the difference lies in directory hierarchy organization which that is of secondary importance.
But I believe Marco when he says atomic operations do not give you guarantees in the strict sense of the word.
