I'm using dvorak systemwide.
Here's a grep on dvorak an my /etc dir (excluding the keymaps in X11
directory)

/etc/X11/xorg.conf:36:Option "XkbLayout" "dvorak"
/etc/kbdtype:1:us.dvorak

What I did was during the initial install of this machine I gave
'us.dvorak' as keyboard layout.
