I have a ThinkPad T60, and initially had the same problem.  Just set the 
volume to its maximum output with the command

 mixerctl outputs.lineout=255,255

and then press the hardware volume buttons (located next to the blue 
"ThinkVantage" button), to adjust the volume to its desired level.
