The setup I have been using for a long time now is based on this document: http://postfix.wiki.xs4all.nl/index.php?title=OpenBSD_PostfixAdmin_Guide I've succesfully used that on everything up to (and including) 4.1, although there are some small changes. Namely the courier-auth libs are now different packages.

However I have never liked the sasl2 / imap connection setup - it seems a bit dodgy.
Then again I am not anywhere near able to come up with something better.

Any new mailboxes / aliases / etc are immediately available, which is nice and convenient.
Downside is MySQL has now become a point of failure for maildelivery.
