I have an external machine, and i try my setup from this external machine
that enters directly to $ext_if, so direction is IN.

Are you *certain* that your traffic is not being redirected properly?
Verify this with a tcpdump on your internal interface to see if the
packets do get routed.

You may also increase debugging in PF by running 'pfctl -x loud' and
watching your kernel log (typically /var/log/messages). When you're
done 'pfctl -x urgent' will return your debugging to normal.
