I am using a very old Lenovo x120e that is suffering of a
similar issue.  On a fresh boot the speakers work for several
seconds (maybe a minute?) but then go silent.  The headphone
jack is the only reliable means by which to listen to audio
on this thinkpad.

I may have posted about this many years ago, but I'm not sure.
