Here is one suggestion . . .

1) Use sendmail that is installed from the stock OpenBSD CD, just comment 
out the localhost.cf portion in rc.conf so that you use the standard 
sendmail.cf

2) Edit the virtusertable in /etc/mail (follow the instructions in that file 
to suit your requirements) and add entries for each of the domains you want 
to accept mail for and the servers or accounts you want to send them to

3) If you want to accept mail for multiple domains on the same OpenBSD 
Server, then add those domain names in /etc/mail/local-host-names file

4) Uncomment the entry for pop3 in inetd.conf and you will have a good, 
working, pop server

You could use imap with dovecot or whatver but with these four steps you may 
be able to address all the issues you mentioned without having to rely on 
any additional ports or packages.
