The debug=wm output looks reasonable to me.  Only funky thing I see at the
moment is the gl_FogFragCoord output from the VS not getting dead code
eliminated, but that should be fine (looks like output/input mapping works
correctly when testing that standalone).

Unfortunately, replay on softpipe is crapping out for me (incomplete
framebuffers).
