I would like to know what other hospitals are doing when a Medicare
patient is re-admitted within 72 hours of discharge with the same
admitting diagnosis.  
     1. Are you keeping the 2 admissions with 2 separate account numbers
and just billing them as 1 admission.?
     2. Do you undo the first discharge when you get the 72 hour warning
and use the first account # for the re-admisssion 
         hospital stay?
     3. If  the patient is admitted with a new account # and 3 days
later you are told the account has to be merged, are you 
         merging the clinical documentation?
     4. Is there another way to handle this situation?
