If so could you answer a few questions? 1) Which medication dispensing unit are you using? Pyxis 2) Are you live with CPOE/POM in EDM? Yes .. close to 100% of orders entered by MD or PA. Nurses occasionally enter orders. 3) Are you using eMAR in EDM? Yes (Dispensing machine override orders are blocked from the eMar. Nurses must have an order to complete eMar documentation.) We originally allowed overrides to go to the eMar, but there was too much duplication .. Nurse took out med as an override, and MD entered order at roughly the same time. The result was that there were two orders for the same event on the eMar. Now we have some EDM orders with documentation of administration but no charge since the charge comes from the override. We monitor this via an NPR activity report. We are currently set to bill on dispense .. eMar is currently only live in the ED so Admin billing would be a problem because so many patients are admitted and there is no clear delineation of what orders are EDM and which are PHA. 4) Do your staff like, or dislike, Profiling in the ED? Staff like it extremely well. Also is a big improvement med safety. Nurses are directed to the item ordered, not an item pulled from the override list. 5) Have your staff experienced delays in med administration due to Profiling? Not usually. If there is an interface problem, then the override function is available as a back-up. Orders appear on the Profile in 30 to 45 seconds after filed in EDM. (Pyxis polling is set to 30 seconds). Also, there is some delay in the MED indicator light displaying so by time the MED light is visible, the order is usually on the profile. Nurses now hate to use the override so there is more incentive for MD's and PA's to use the system via order entry to control the medication process. In critical emergent situations, Meds are sometimes removed on override, and orders entered after the fact. eMar documentation occurs after the orders are entered. We also have a few lock boxes with emergent meds that are replaced via order entry and subsequent withdrawal from Pyxis and documentation on the eMar.
