We are currently doing this using Iatric's POI. It is a big problem for the physician practice if their patient names don't match ours and is a source of frustration to them. I supplied the physician office with our hospital admissions policy on the convention used for registering patient names. This was to let them know that if they have a patient called Jim Van Doe, we would register the patient as VANDOE,JAMES X. This cleared up some of the problems.
