eMAR Rules: Flag for PRN too soon from last dose

PHA DTS 6369 gives the ability to create eMAR rules that can be
evaluated for MAR DOSE and MAR ADMIN.

Many sites would like to create a rule to flag nusing users if they give
a PRN medication too close to the last dose given. The following is an
example of a MAR ADMIN rule that will flag users if they try to give a
medication with the direction of PRN/Q4H more then once within 4 hours
from the other dose:

Direction: PRN/Q4H

Rule:

 [f z last admin],[f z today/now in sec]^NOW,
 NOW-ADMINSEC^TS,
"It has not been 2 hrs since this med was last administered"^MSG,
 IF{([f rx sig]="PRN/Q4H")&(TS<7200) [f rx err msg](MSG);1};

Below is the information on how to build the custom keywods "z last
admin" and "z today/now in sec" :

 z last admin
Active: Y Assign to Group: Z
Name: last administration time
Multiple Output Lines?: N
Execute at Translation?: N

Code:
""^END,
DO{(@Last(admin.q)^admin.q)&(END_.=.) @admin.seconds^ADMINSEC,1^END}

z today/now in sec
Active: Y Assign to Group: Z
Name: today/now in seconds
Multiple Output Lines?: N
Execute at Translation?: N

Code:
%Z.date.clinical(@.today)*86400+%Z.time.in(@.now)

variances

Yes, this was an issue for us too. We had to really explain the message
in training because our users found it confusing. Our previous barcoded
medication administration system had separate parameters to warn the
user if a med was too early or too late. The warning message stated "too
early" or "too late". We could even set a prn parameter that warned if
the med was "too close". I would like to see Meditech change their time
variance to separate customer-defined parameters like we had with our
other system.

Subject: [MEDITECH-L] Issues with new eMAR verbiage re: time
variances

Dear L'rs - We recently upgraded to 5.6 and are experiencing a
great deal of confusion with the verbiage Meditech chose to display
related to time variance warnings. In short, the wording makes many of
our staff think they are choosing a time ahead (when it is really late)
and vice verse, unless they really read and analyze the message.

Anyone else encountering issues with this with their end-users?
We are considering asking Meditech for a custom if that is the only way
to resolve, but would love to hear from other sites about their
experience. TIA!
