We also use the 4 mg tablets. We have an Order Set with all of the admin times. By combining some of the repeated doses/times, I was able to trim it down to (only) 7 orders. We have the Start Time hard-coded to the first dose of the first day. That works fine if the prescriber orders the Dose Pak to start the next day. We have a Label Comment about "catching up" if the order is to start now, which is always after that first dose was due. Of course, that doesn't work seemlessly with eMAR. We are currently rolling out eMAR, and I am not aware that this issue has come up yet in practice. I suppose it would be possible to have the Pharmacist customize the first day's doses, but that would mean I'd have to re-build the Order Set, or create another Order Set, because some of those orders include the first, second, and third day in the same order. And for that scenario, don't even think about POM! I don't see any other option but to have the Nurse document the "missed" doses and the reason for the "extra" dose on that first day. After that, the doses and the times will be correct on the profile and the eMAR.
