I am posting this for our Business Office: We are currently in the process of billing for a Physician Assistant in the Emergency Room and would like to know if anyone is billing for them and how did they set up the 1500 claim form. We know they must have a supervising physician listed on the claim so what field are you putting that information in? In locator 33 we need to list the hospital as we are doing the billing. If anyone in Colorado is billing Medicaid let us know how you are reporting the physicians as they need a supervising, Primary Care and the rendering.
