We have a listing for UNDEFINED and have a CDS attached when this is used through Admissions or Scheduling that requires them to gather certain information. A form is faxed to the physician's office for completion and to be faxed back to the Medical Staff office. This information is also faxed to the Medical Staff office to contact the physician for further information needed if the form is not received from the physician's office.
Unfortunately, the process is not followed completely at all times. Radiology personnel have been taught to change the ordering physicians name at the time of the order entry and free text in the correct information so reports are sent correctly. Delores P.
