Hi Terry,

 

I wrote a report that pulls clinical information from all the nursing
documentations; this report provided to the receiving department when a
patient is being transferred between departments or when they undergo
diagnostic procedures.

 

It is very complex, since it is pulling from many interventions that may
be documented at different points in time, and as such uses many
fragments.

 

I included in this report besides the basic patient identifiers, such as
first name, last name, medical number, visit number, location, room,
bed, date of birth, and age, also a lot of clinical information, such
past medical and surgical histories, current diagnosis, recent
procedures, allergies, isolation status, height, weight, pain, neuro
status, and vital signs.

Is anyone using a Nursing Hand off communication tool (when patient is
transferred from one location/service to another)?  If so, what are you
using?  

One of our facilities is using a OE ADM screen (they do not want this to
be part of the patients permanent record) in the ED.  But now they are
looking for a way to communicate from the Ancillary departments (Cath
Lab, Rad, etc).

Any help is appreciated.
