Aimee, it also happens to us sporadically.  We have not been able to
figure it out, either.  Anyone have ideas?

Another issue:
 
Since we went live with Pyxis profile we have issues with the dispense
from Y field disappearing from the inventory in the drug dictionary.
This means the refill list lists these drugs and the pharmacist has to
credit the patient so they are not double charged. 
 
We've had a task open with Meditech but they haven't found a fix. We
turned on the dictionary audit trail for PHA.DRUG and PHA.INV but since
it does not show old and new values it really doesn't help. It only
shows how many times Pharmacy staff has had to go in and re-enter the Y.
Has this happened to anyone else?

All of our inpatient areas are on full Pyxis Profile, with no carts
except PBs and IVFs. We have some Physicians using POM, in which case
the orders must be Verified by a Pharmacist before they pass to the
Pyxis. The majority of our orders are still written in the chart, faxed
to the Pharmacy, and then entered into Meditech by a Pharmacist. Those
orders pass directly to the Pyxis. The only orders which, by policy, are
double-checked by another Pharmacist are high risk meds (Chemo, TPN,
etc) which are not dispensed from the machines. I'm not sure that
Meditech even has a mechanism for Pharmacists to verify Pharmacists,
unless you give each Pharmacist two separate login ID's: one for Not
Verified and one for Verified.
