Hi All, we are in the process of implementing LTC. Here are our questions. Any insight is appreciated!


We had a meeting this morning and here is our list of questions thus far. Any insight or feedback would be greatly appreciated.  Thanks! 

 

Bed Hold - Is this function being utilized? If YES, please proceed to the alphabetized list of questions
       

How are Bed Holds being handled for Residents when they are sent to a hospital and then admitted? Are pay Bed Holds or non Pay Bed Holds being utilized?

 

If utilizing Bed Holds how do you calculate the Admits/Discharges for Medicare Cost report Worksheet S3?
 

If a patient is placed on a Bed Hold and then does not return to the long term care facility, what is being entered as the discharge date?

 

Liability - we are planning on utilizing the Bed Holds in order to best calculate liability since the liability is for the month and not for an individual visit.  How are other health care organizations handling this?

 

 

Reports - Are standard reports or Custom NPR reports being utilized to collect the following information:
 

Need a report to display each resident along with ALL Applicable Insurance Contacts (not just the most recent) especially when a resident has multiple insurance contracts.

 

 

 

Billing Questions- How are the following scenarios handled?
              

If the resident changes insurances (for example to or from Medicare) how is the reclassification handled and how are the Admits and Discharges entered into the Medicare Cost report worksheet S3.

 

b. How are Part A therapy charges getting posted to a Bill? Are they only being grouped by the last day?

 

              

Miscellaneous Questions regarding policies and procedures when sending a Resident off site:
                

              a.   What info do you give i.e. a Doctor's office when a resident is treated by that doctor??  We need to give them face sheet/ demographic info and also the Insurance Information that Doctor's office needs for billing.  

              One of the major questions we are trying to resolve is i.e. when a Resident no longer has Medicare Part A effective for their Nursing Home Billing  (may then be Self Pay) -yet they do have Medicare that would pay for the Doctor's visit - how do we get the Doctor's office the Medicare Info??

               Also, how are you communicating to the Doctor's Office (or any ancillary) whether their charges should be billed to us to include in our Part A billing or that they should bill themselves under Part B ??
