There shouldn't be any limit (there was a limit in pre-MapServer 5 versions, of a MAXLAYERS, but that was removed).

For testing I always use shp2img commandline utility on the mapfile, to get the exact error, with the switch:

   -map_debug 3

That should likely return an error for your problem index record like:

   Unable to access file.  \ms4w\apps\tileindex\ttt.tif

It might also be good to share your exact error here to everyone. (if it is a lot of text, be sure to paste the link instead here, from https://pastebin.com/
