The technical side doesn't seem too hard, but the political side would 
probably be "interesting".

What fraction of lists/traffic is currently not explicit opt-in?  The case I 
see is getting added to a deal/spam-of-the-week list when I buy something 
online.

I've seen various comments about junk mail after registering at a site.  I'm 
not familiar with that area.  If a site wants me to register, I generally go 
someplace else.  (Yes, there are a few exceptions.)

How many of the bulk mail senders would cooperate?  Would they be willing to 
switch all their customers to real opt-in?
