I see something similar when printing my iTunes Receipts for instance.
Background colours etc. aren't printed. That is consistent (or I'm
missing something).

It always works normally for me…
If I have an image with external/unloaded images, then click the
Load Once button, then print, the images are included in the printed
version as expected.
Sometimes that works; more often, it doesn't, and I'm not sure why.
Let me know if you spot the pattern. I couldn't reproduce it myself.
