There are several things that could cause this.
1. Though unlikely, you were pressing command+W instead of Command+Q to
quit Mail.
2. You closed Mail with several windows not just the main inbox window
opened.
3. You have a corrupted Apple Script, which is playing havoc.
4. You had your vo keys locked with vo+semicolon, thus actually causing
Voiceover to navigate to next block quote, which normally is vo+command+q,
although this is probably unlikely, seeing that you hit a menu bar.
5. You were really stupid, and changed the pronunciation dictionary entry
from saying quitting mail, to saying menu bar instead, then attached it to
an activity for mail.app
6. Or, the absolute most likely cause, you have Voiceover set after a
certain amount of time to read what is under the mouse, and your mouse
pointer at the time of quitting mail, just happened to be sitting on the
menu bar, thus, triggering that announcement.
The only other thing I know of would be a corrupted plist file, though
again, this is very unlikely. The other thing is that you were trying to
quit by a mapped keystroke which has been mapped to a key combination on a
braille display which isn't working correctly.
Or, you had a wireless keyboard, be it bluetooth or not, and either the
batteries were getting low and causing unwanted results, or the transmition
in general was not being properly interpreted. The only other thing I could
think is that you have a Windows keyboard connected, and were pressing what
you thought was command+q when actually, you were pressing option+q, as
remember, the command and option keys will be reversed on a Windows keyboard
plugged into an Apple machine. Furthermore to expand on that, you also in
addition to having those keys reversed would have had to have a keyboard
commander set up where by option+Q opens your menu bar. I dono why you'd
ever set it that way, but each is to his or her own, I guess.
Those are the only reasons I can think of. the only other is your
schizophrenic and are hearing voices that aren't there. No, just kidding.
