The only other thing I can suggest is to try moving across the download column 
with mouse keys turned on. Mouse keys will let you move your mouse by keyboard 
controls to locations that VoiceOver cannot reach. You have to set "Speak text 
under mouse after delay" to use this.  Under VoiceOver Utility, go to 
"Verbosity" with Command-2, then select the "Announcements" tab. Check the box 
for "Speak text under mouse after delay", then interact with the slider and set 
the delay to 0 seconds. Close the window with Command-W.

When you're on the web page, navigate to place your mouse cursor close to where 
you think the download button will be -- either to an item in an adjacent 
column, if you're going to move horizontally, or to the column heading, if 
you're going to move down vertically.  Turn on mouse keys in System Preferences 
> Accessibility (or Universal Access for systems before Mountain Lion).  In 
Mountain Lion you can use the Command-Option-F5 shortcut instead of going 
through system preferences.  I have this set up so I can toggle mouse keys on 
or off by pressing the Option key 5 times.  Then hold down the FN key to use 
the K, U, I, O, and 8 keys to control the mouse when mouse keys is on.

The "i' key sits in the middle of the 9-pack of keys for the old embedded 
numeric keypad, where "7 8 9" in the top row number keys matched the "7 8 9" on 
the keypad, and "u i o" matched "4 5 6", with "j k l" matching "1 2 3". 
Pressing the key to the right or left of the "i" key central "5" position moves 
the cursor one pixel in that direction with mouse keys on.

If you hear VoiceOver announce "download" or something similar when you move 
over a location with mouse keys, you can click with the cursor by pressing 
FN+I.   (I don't have to use the FN key for mouse keys on my laptop, but I 
think you have to press the FN key if you're using an extended keyboard with 
numeric keypad, so in general, use FN+K, U, I, O, or 8).

Mouse keys are a way for people with motor disabilities, like tremors from 
Parkinson's disease, to achieve accurate fine control of mouse positions with 
the keyboard, so you generally need to do a lot of key presses -- like 50 to 
100, or more. If you do a lot of work with mouse keys, you can use VO-F5-F5 or 
VO-F5-F5-F5 to report the position of the mouse relative to the screen or the 
current window, and then check this against the x position of your column 
header, or the y position of your starting point in another column, also 
reported with VO-F5 twice or VO-F5 three times.  Have VoiceOver report in 
pixels rather than inches or millimeters if you use this -- change the pop up 
menu button on the Announcements tab under Verbosity for "Speak size and 
position in" to "pixels".  This is the control just before the "Speak text 
under mouse after delay".

HTH.  I use mouse keys either to click (FN+I), or to move to problematic 
locations in web pages or applications with accessibility issues.  (This 
includes having to do any set ups with "Java Preferences" on the Mac, sad to 
say.)
