I don't know much about what's under the hood but yes, Firefox is open
source so you could download the code and poke through it, even
contribute bug fixes to the project. That said, anybody who has that
level of skill at writing code for complex multi-platform apps is
probably already swimming in job offers and requests for help. I suspect
it's just one of those efficiency things where the initial
implementation is simulating the entire molecule instead of the atom or
collapsing a kingdom to save a nail. Pick your euphemism. It's easy to
get inefficient with complex stuff and that gets weeded out over time.
First they try to just get it to work, then work reasonably fast.

in one of the two codes find what is causing the breakage.

I believe it is available. Firefox is usable with VoiceOver. It is a
bit sluggish in some instances, but it is getting there. If you have
patience, I would give it a try and encourage the developers to keep
improving VoiceOver access.
