I've text you a couple times this week, but didn't get an answer.  I tried
to install on my iPhone5, and it locked up on me like yours did, and this
app was never there.  Interesting.

Hi yes, it works for the call back method. but not for direct dial. So I use
a combination of call back method and auto answer. With auto answer I can
set my own gv number as the incoming number which gets auto-answered. So,
when I place an outgoing call, it does the call back, making my phone
receive an incoming call from my gv number. Auto answer does it's job and
automagically answers the call, and the outcome is a seamless outbound call
like everyone else, but coming from my GV number. 
It works like a champ. As to why this tweak didn't work on the other iPhone,
my guess is that since I saved my settings, installing it on my own i Phone
made it read the settings file and somehow that prevented it from locking
up. However on the other iPhone which never had Phone GV Extension, maybe
there was no settings file to read. So, if I find where the settings are
kept, maybe I'll transfer that information to the other phone and then
install the extension to see if my guess is right. Who knows, more to play
with. At least I now know how to get it out of this zombie mode in case I
screw things up again. <grin>. I love this.

phone and jailbroke again, however that takes a lot of time. So, all you
have to do is first reset your phone the regular and Apple approved way by
pressing and holding Home and Power/Sleep buttons for around 10 seconds.
Then release the buttons and wait for the reboot to happen. Now, this might
fix your problem, but if it doesn't, then boot your device in safe mode and
then you can go into Cydia and remove the app or tweak which caused your
woes in the first place. To do this, reset your phone using the method I
described, while it's booting up, press and hold the volume up button. Do
this until the phone boots and Voice Over speaks. It will now be in safe
mode. Now you can open Cydia and remove the last app or tweak you installed
and then reboot normally to be out of safe mode and the Apple should be
