Yes, the Mac version of Paprika is accessible.  In fact there's a
review of Paprika on the Mac at the AppleVis site, and no review of
the iOS app there, even though the accessibility updates for the
versions for iPhone and iPad (there are separate apps for each), were
completed before they finished updating the Mac app.  And the comment
in that review about wanting to set the app into text only mode
because you can't use VO to select recipes in the photo and text view
mode hasn't been true for several months.

Yes, you can check off items in the grocery list as you use it.  You
can check or uncheck any of the ingredients in a recipe when you
create your grocery list, and you can also add other items. Your
grocery list is viewable in three tabs: "al"l, "bought", and "to buy".
As you check off items, the lists in the different tabs gets updated.
You can email the list, but I don't think there's any standard export
format for your grocery list.

I think the strongest point of Paprika is really the cloud syncing
with iOS devices. So it's perfectly possible to just get the iOS apps,
load any recipes you want on your iPhone, and use it to browse recipe
sties for new recipes you want to add, or input your own, without
getting the Mac version, as a number of iPhone users who use Windows
do.  If other family members have the app, you can sync your meal
planning, groceries, etc. through the cloud account that comes with
the app.  What's nice about the Mac app, is that the cloud syncing
between the Mac version of Paprika and the iOS versions works.  There
are other good, mature, recipe apps for the Mac, such as MacGourmet,
and MacGournet even has an iOS app.  But what I read in the reviews is
that many MacGourmet users complained that cloud syncing with their
iPhones didn't work, and so they moved to Paprika, which can import
recipe formats from other apps.

The main reason I got the Mac version of Paprika, apart from the fact
that it was discounted to half price for "Black Friday" weekend, was
that I didn't want to add recipes that were already on my computer by
having to connect my iPhone and use iTunes sync.
