Hi Steve. You don't have to have the voices change pitch or say cap. The way I got it set since I'm currently using Cereproc Sarah, (which has become my favorite female voice because she's the only female voice that takes breaths,) since she can't do pitch changes, I have VO beep when it encounters a capital letter. Have you thought of giving that a try?
