Chris, I don't think there is a way to set Command Tab as a custom braille command since it is a Mac-specific command as opposed to a VoiceOver command. as for setting up a custom braille command, after you hit the Add Button, find the key you wish to assign and fit Command-B on your keyboard. You'll have 5 seconds to hit the keystroke you desire. If you are modifying an existing key, you will have the opportunity to overwrite the key. Hope that helps.
