Regarding your second question, I saw this issue wants. What I did was brought up a list of all of the open active windows: FN, control, option, F2 twice quickly This is on a MacBook. One of the options was, something to the nature of my Mac had shut down to repair an error, or something like that. I press return on that particular window, and then vo over to cancel. The window was asking me if I wanted to reopen all active windows that were open previously to my MacBook shutting down. This could be what is going on for you. Whenever I was using any keystrokes voiceover was stating "dictation IM window". I am not sure if this is the same thing that is going on for you, but check it out. Thanks
