I think RipIt grabs everything. If you do the compress option I think it will drop it into iTunes but I've never tried that.

According to Apple Discussions, if you hold down the Select button (center of the big circle buttons), it will bring up your chapter selections. I've not tried this with VO but it does appear to work in the sighted world.
