Hi Chris -- I have experienced this a bunch of times recently on my 2010 MBA. I've gotten so used to turning VO off and on several times a day that I hadn't even bothered to try troubleshooting the issue. I don't think I've seen it happen since updating to 10.6.8, but it's only been a few days...
