Hi Erik - I could be wrong, but I was under the impression that there were no 
settings to permanently enable or disable switching to speakerphone. During a 
call or when checking voicemail, there is a button labeled "speaker" that can 
be used to toggle the speaker during that call, but I'm pretty sure that only 
holds for the current phone call. Did your client perhaps install a new case 
recently or have they done anything else that might be covering the proximity 
sensor? If that sensor is covered, even partially, it could cause the phone to 
stay out of speakerphone mode. The sensor is located above the earpiece on the 
iphone 4, and next to the earpiece on the iphone 3GS.
