Well, once you've selected all, Shift-Arrow adjusts the selection from the start of the document. Alternatively, if you have no selection and you navigate somewhere in the page, then press Shift-Arrow, it will start selecting from that point. VoiceOver may not always do a perfect job of communicating what's happen though.
