Well, once you've selected all, Shift-Arrow adjusts the selection from
the start of the document.

Alternatively, if you have no selection and you navigate somewhere in
the page, then press Shift-Arrow, it will start selecting from that
point.

VoiceOver may not always do a perfect job of communicating what's happen though.

If it's not working, or if VO is not communicating what's happening, I
think that a bug.
