I believe you are asking if there is a way to assign a key sequence to 
automatically launch a specific app.  For instance, I have a Commander using 
the “w” to launch MS-Word.  When I wish to use MS-Word I type in the key 
combination of <Right-Option-Key><w> and the app is opened.  Now for some 
explanation (WARNING, this may turn into a dissertation):

If you are familiar with the VO Utility and Commanders, stop reading now!  Or, 
if you prefer, use the online help feature to familiarize yourself with the 
VoiceOver utility and creating Commanders.

First, to see the list of existing Commanders press <VO><hh> (yes, that is two 
h’s to press quickly in sequence while <VO> is pressed), this will place you in 
a menu.  Arrow down in this menu until you get to “Keyboard Commanders.”  Then 
arrow right, you will find yourself in a list of existing Commanders; each line 
in this list will identify the function (i.e., open application), the app and 
the key combination to activate the Commander.  For example, in my list I would 
find “open application, Word, <RightOption>w

You can use <ESCAPE> to get out of this.

Now to create a Commander:

1) Open the VO Utility (<VO><F8>
2) You will find yourself in the “Utility Categories” table; arrow down until 
you observe “Commanders"
 3) At this point, arrow right whereupon you will be in a set of 3 options to 
select where the “hot key” will come from.  In my case I use the “w” from the 
keyboard so I select <KEYBOARD>
4) <VO><RightArrow> and make sure the “Enable Keyboard Commander” checkbox is 
in fact checked.
5) <VO><RightArrow> to the “Use” Pop-Up, activate and make your desired 
selection from the list.  In my case, I use the Right Option key.
6) <VO><RightArrow> past the “Commanders Table” to the “Add” button and 
activate it.  This will place you in that “Commanders Table” where you will 
enter the desired key to use as the “Hot Key” (in my Word case, I’m using lower 
case “w”).
7) After entering the desired letter, <VO><RightArrow> where you will find 
yourself in a menu and select “Custom Commands” and arrow right there.
8) Now you are in the “Actions Menu” and arrow”Open Application” should be the 
first option, select it.
9) By default, the above action will place you in the “Applications” folder 
where you may select the desired program or move to an alternative folder where 
the desired program has been installed.

Once you have selected the application to launch with the assigned “Hot Key” 
you can conveniently use that “Hot Key” to launch that application.

As I mentioned earlier, when I want to open Word I just enter <RightOption><w>

NOTE: the identified letter is case dependent.  If I so choose, I could create 
a Commander using the upper case W, this would be different from the lower case 
“w” Commander.
