Hello Kate, The problem you described has been bugging me for months, on my 8 Plus, also.
After reading Pete's subsequent reply on this, I discovered that in order to get speaker phone activation to work, I had to first disable the Select Speaker setting, in VoiceOver options, then re-enable it.
