Basically, the WatchOS is not as robust at this point as iOS, iPadOS or MacOS.  
When adding languages, it appears that you are adding the default base voice 
for that language.  So, if you added Australian, you’d have Samantha for the US 
English default voice and Karen for the Australian voice.  If you added British 
English instead, you’d have Samantha and Daniel.  Now, since you added US 
English, you have Samantha and Samantha :).  An extra clarification though to 
your understanding.  Donna did not simply enable Siri on her watch, what was 
suggested was to tell the watch to use the Siri voice instead of the base voice 
for the selected language.  This setting is found in Settings, Accessibility, 
VoiceOver.  This setting makes it so that your selected Siri voice is used for 
VoiceOver speech.  In my testing, when this is selected, switching English 
languages using the rotor will not actually change the voice though, as the 
Siri voice seems to have taken control.  I’m not sure though if this is the 
case if you choose a non-English language as your alternative.

Does anyone know why this does not work?  

Again, I am asking because switching between these two languages and their 
respective TTS engines on my phone works so well.

I finally got it to work.  I had to power the watch off and back on, and then 
the option to turn on the SIRI voice worked.

I feel your pain re sleep.  When my watch self-destructed, it was four AM, and 
of course I couldn’t go back to sleep because I was trying to figure out what 
was going on.  Have you tried Melatonin?  I Take the 10MG tablets, and they 
help a lot, (barring self-destructing watches).  Just FYI, for me the gummies 
are less helpful.  I think it’s because they get into my system too fast, and 
so don’t help me go back to sleep when I wake up at 3 AM.  Just a suggestion.
