First, in VoiceOver Utility, under the VoiceOver Recognition category, make sure that Image Recognition is enabled. You can download extra languages as well if any of your images require translation or to be spoken in another language. Next, open the pdf in Preview and navigate VO to the Document area where the image is. Finally, press VO-shift-l. This should do the job. I tested this on a screen shot and it worked, sort of. The first time I pressed VO-shift-l, VO said “Preview, Not Responding”, then said “No Description Available”, Thus I said “Hmph”. So, being persistent and all, I waited a few seconds and pressed VO-shift-l again, and low and behold, it read out the text on my screen shot. It did a wonderful job too. HTH.
