You can use the Keyboard Commander in VoiceOver Utility to associate key combinations with commands to start applications.

You can also change the language in both the VoiceOver speech and braille preferences. However, in some cases, VoiceOver detects the language being used and adjusts accordingly, for example on Web pages.
