Hello Ramy, My preferred medication reminder app for iOS is called Pill Reminder. There is both a free and paid version. It is 100% VoiceOver accessible and completely customizable.
Once configured, you need not open the app in order to get your reminders. I have been using it for about 6 months or so and have come to greatly depend on it.
