Likely you are correct. I need to go back and see if I set up something incorrectly. It’s strange that a couple files are updating to fairly recent vintage, but most of them are stuck back in the dark ages.

If it's not a brand new thing, I have seen it take a while for the iPhone to see updates or changes to files. Not a real long time, maybe 5 minutes or so. The only way I know to refresh things, would be to either turn off iCloud Drive on the iPhone, then back out of Settings, wait a minute or two then re-enable it. I don't recommend that approach. Alternatively, you could close the Files app from the app switcher, then upon re-opening it, it would do a poll of the existing iCloud Drive data. Again, this should only be necessary if you've been syncing iCloud Drive for a long time and this is just happening lately. Finally, of course, there's always the good old restart of the iPhone that likes to fix all sorts of unknown issues. HTH. Later...
