Petrus, It sounds like the external Bluetooth speaker is messing with you. The iPhone has multiple channels for sound including the internal speaker, , the earpiece speaker, headphones and external AirPlay or Bluetooth sound. Normally, these can be adjusted independently but I believe the confusion starts to manifest itself when Bluetooth speakers come in the mix. It seems that when the Bluetooth speaker is connected, often the ringer sound is routed through that Bluetooth speaker when a call comes in. You'll also note that when a Bluetooth speaker is connected, modifying the volume only affects the output volume of the Audio Book, music or streaming audio. Adjusting the volume does not change the volume of VO if VO is still routed through the internal speaker. So, I guess what I'm saying is that it's complicated when you start having multiple audio sources going at once, especially when you add VO sound to the equation. I guess that you could say that I'm not really giving a good answer here, just a bunch of excuses, but I'm just attempting to enlighten you as to the complexity of the audio routing behaviour when all these factors are involved. Whether it should or shouldn't behave a certain way when the Bluetooth speaker is connected, I'm not sure, but when there's a not a Bluetooth speaker involved, you are correct in how you believe it should behave.
