Try activating window chooser menu and check whether there is a window asking 
you to enter your computer password to proceed.  That has sometimes happened to 
me in other situations.  I don’t seem to be able to proceed because I need to 
enter my computer password but Voiceover doesn’t report this.  Have a go in 
case this is true for you.
