Thanks Tim... Unfortunately the fields in the document were not labelled so preview was not useful. I will go back to the creator with this information.
Have you used pdf pen? Do you know if it is accessible on the Mac or in iOs? The apps seem expensive but ... if they are accessible then they may be worth the investment.
